ABOUT THE JOB: Open to Philippine residents ONLY! One of our US-based clients is looking to hire a Home Health Scheduler to manage, maintain, and update schedules of US-based medical/healthcare providers going around to each patient's home. RESPONSIBILITIES: Job duties may be modified based on experience and fit within the company. Admin task for the Diabetes Management Program (DM Power)Answers all calls for queries and info about the programGet internal leads/patients for DM PowerEnroll patients to DM Power and RPM HTN (Relay and explain all necessary information)Updates patient's chart and SalesforceScheduling patients for coach sessions and provider follow-upOrders all necessary supplies needed for DM PatientsInitial linking to devices.Troubleshoot devices primarily for BP devices and in some cases for DM devices. MUST HAVES: Experience with Diabetes Management Program (DM Power) is REQUIRED .
