Description:
The Medical Admin-PHRN is in charge of providing remote patient care support and administrative assistance to assure the accuracy and quality of healthcare services. This role requires a senior-level nurse with a solid clinical experience who can effectively monitor patients, do quality assurance checks, and examine patient charts to identify at-risk individuals.● Document any changes in patient condition on a daily basis, including vital signs, symptoms, andtreatment responses.● Communicate relevant updates to the healthcare team and escalate concerns as necessary.● Review patient diagnoses to ensure accuracy and appropriateness of treatment plans.● Conduct regular assessments of medical records and documentation for compliance withestablished standards and protocols.● Identify at-risk patients based on medical history, current condition, and other relevant factors.● Perform comprehensive chart reviews to assess the adequacy of care and identify opportunitiesfor intervention or improvement.● Collaborate with healthcare providers, clinicians, and administrative staff to identifyareas for improvement in documentation procedures and maintain consistent qualityacross all records.● Utilize extensive clinical knowledge and experience to assess patient needs, prioritizeinterventions, and optimize outcomes.● Review OASIS assessments and document recommended changes in the OASIS database tool● Review ICD-10 coding and sequencing from documentation in the patient chart● Enter results from review in appropriate fields of the database● Provide explanation for change and reimbursement impact recommended Field Notes, Chart Review● Be open to accepting new challenges and tasks as they arise. Requirements: ● Active Registered Nurse (RN) license.● Excellent communication and interpersonal skills.● At least 2 years of experience in US Healthcare Account● Minimum of 2 years of clinical experience in a healthcare setting.● Proficiency in medical terminology and electronic health records (EHR) systems.● Experience in Homehealth or Hospice setting● Effective communication skills, both verbal and written.● Ability to work independently and collaboratively in a virtual environment.● Excellent attention to detail and organizational skills.● Strong analytical abilities with the capacity to critically evaluate medical information.● CAN START ASAPPowered by JazzHR
