JOB DESCRIPTION POSITION: Recovery Support Worker/Intake Worker REPORTS TO: Behavioral Health Clinical Coordinator STATUS: Regular Full-Time SALARY: NegotiableRESPONSIBILITIES: Conduct Recovery Support Evaluations with clients, creating individual treatment plans based on ASAM placement levels. Contribute to the development of integrated treatment plans and discharge summaries. Document clients' recovery support service needs, develop comprehensive individual recovery support plans, and monitor progress every ninety (90) days. Engage clients through outreach and motivational interviewing to ensure active participation in the treatment process. Update individual recovery support plans every 90 days, aligning with ASAM placement levels and client needs. Complete follow-up contacts for No Shows and post-completions, ensuring thorough documentation. Facilitate recovery support sessions and assist with assigned drug screens. Possess in-depth knowledge of community resources and the ability to cultivate new ones. Maintain up-to-date documentation in electronic data systems for clients and collateral contacts, locking notes within 24 hours of encounters. Assist in client chart reviews, both paper and electronic, data collection for quality improvement (QI) protocols, and preparation of required monthly data and program reports for grants and contracts. Participate in scheduled supervision, staff meetings, training activities, and maintain certification/licensure. Attend regularly scheduled case conferences, staffings, site visits, and multidisciplinary team meetings, presenting assigned cases. Perform any other duties as assigned. HOURS OF WORK: Monday through Friday, 8:30 a.m. - 5:30 p.m.; evening hours as assigned.QUALIFICATIONS: Recovery Coach with a bachelor's degree from an accredited college or university in social work, counseling, psychology, or a closely related field. Training or relevant experience in substance use. Alternatively, an individual with at least four (4) years of relevant, qualifying full-time-equivalent experience in human service delivery, demonstrating skills in developing positive community relationships and navigating complex service systems. Appropriate licensure/certification required. Proficiency in data entry in electronic data systems is necessary. Desired experience in HIV/AIDS.
